Our Dementia & Chronic Disease Care Program offers a holistic care plan for managing chronic conditions such as Alzheimer's, COPD, CHF and Stroke. We assess the whole person and create a care plan that matches the unique needs of each client.
As a proud member of the Struthers Parkinson's Care Network, our caregivers receive specialized Parkinson's training, education and care guidelines.
Our Transitional Care Program is focused on clients who have been recently discharged from the hospital or skilled rehab facility. Our care reduces the risk of re-admission to the hospital.
