As a proud member of The Parkinson’s Foundation, our caregivers receive specialized Parkinson's training, education and care guidelines.
Transitional Care
Our Transitional Care Program is focused on clients who have been recently discharged from the hospital or skilled rehab facility. Our care reduces the risk of re-admission to the hospital.
